      Routine Office Visit $35 (without any office procedures)
      Physical $40 (does not include urine test)

All office procedures and vaccination charges are in addition to the routine office visit charge as follows:

      Urine Analysis - $15
      Ear Wax Removal - $35
      EKG - $25
      B-12 Injections - $20
      TB Testing (PPD test/Montoux) - $25
      Asthma Nebulization - $25


      Tetanus Shot - $35
      Flu shot (available from Oct-Feb) - $25
      Pneumonia - $60
      Meningitis (Menectra) - $135
